This approach can help drive a healthier America, with fewer errors and less strain on our limited resources. Many forward-thinking organizations already have adopted this care model and discovered its benefits. Some have advanced even further, rising to the level of a technology-based PCMH with results that are path-breaking. In this paper, Sarah T. Corley, M.D., F.A.C.P., and Charles W.
