The 1,592-page proposed 2027 Medicare Physician Fee Schedule arrived July 14 carrying a pay cut, the end of traditional MIPS and the most generous accountable care terms CMS has offered in years, according to the agency's announcement.

Reaction split along predictable lines. Accountable care groups cheered the Shared Savings Program overhaul while physician organizations blasted the conversion factor math. "Once again, CMS has proposed conversion factor updates that fall well short of what it actually costs our members to deliver care," Jerry Penso, MD, MBA, president and chief executive officer of AMGA, said in the group's statement.
