Medicare's proposed 2027 conversion factor for practices outside a qualifying alternative payment model is $32.84, a 1.68 percent cut driven largely by the expiration of the temporary 2.5 percent bump Congress granted for 2026. Most of the conversation about that number is about cutting costs or fighting payers.

There is a third option, and it does not require a single new patient. It is the work your practice already performs, documents in the chart and then never turns into a claim. The scale of it is not small. A study of Medicare claims found that just 3.4 percent of eligible beneficiaries received chronic care management services in 2019. Transitional care management reached about 11 percent of eligible discharges.
